Because the federal government matches every state dollar spent on the program, states do not pay the full cost of expanding benefits. At the same time, every dollar in Medicaid expenditures cut from state budgets triggers more than a dollar worth of cuts in federal funding. These incentives encourage states to expand the program beyond those who are truly in need.
